My dear Oldpulie, with all due respect that is not an answer from an MBBS,MD, PhD!! Puzzling you don't work in that, it is one of the commonest global problems. Besides ischaemic heart disease does not necessarily mean blocked coronary arteries, and one can have perfect coronary arteries and yet suffer from ischaemic heart disease.
Ischaemia is a situation where the muscles and tissues and in this case the heart muscles suffer from lack of oxygen or inadequate blood supply.
Of course coronary artery atheroma with narrowed lumen and coronary thrombosis are common causes. Yet there are several other causes!
Enlarged left ventricle, cardiomyopathy, aortic stenosis, polyarteritis nodosa, Kawasaki's disease, disecting aortic aneurysm involving the ascending aorta and microvascular disease of the myocardium are some other common causes.
It is chicken for an MD, PhD. OK. tell me what are the common causes of left ventricualr hypertrophy and this could be answered by anyone with an MBBS? Last test before we go to another topic.

My dear Oldpulie, 'neer oru periya kallan'!. I am not very impressed with the answers, you being an MD, PhD. Yet some where there.
But there is a remarkable difference between your first set of answer and the second answer. That gives me the clue that someone is prompting or helping. Tell me who is sitting next to you? First answers were complete ignorance but the second set reasonable yet incomplete and important causes are missed out. This appears a group activity.
You could be an Ayurvedic doctor, or a vedha mahathaya, doubtful, you are better anyway. Those days Prof Nandadasa Kodagoda used to lecture every one even in the TV. He taught every thing except his subject.
OK I will still admit that there is some knowledge, so what is your specialty any way, MD is it in what subject(medicine, community health???????).
Before I enter into a serious dialogue with you first I must be satisfied with your credibility. Never mind you are from Batti, I am half Batti anyway. Answer what is your specialty? | Dear Oldpuli, 'Snack' naturally you eat, only snake you avoid often.Of course there are many people who relish snake meat. If you are an MD,PhD, you will know that eating or drinking snake poison do no harm as it is a protein and will get digested, unless of course you inject it into the system direct. Why didn't you tell this to Thalaivar? By the way the confusion between Snack and snake is it an oversight or the learned doctor's language skill?
